FORM FOR UNDERTAKING TO BE SIGNED BY THE APPLICANT
To President of India,
1. In consideration of the president (herein after referent to as Govt) having agreed to my request to permit for the purposed of building of acquiring a suitable house including the cost of site their or withdrawal of Rs 40,000/- (Rupees Forty Thousand Only) from the amount standing to my credit in the AFFP Fund under the Govt of India, MOD letter No 98990/1/AG/PS3(C) 1189/S/G(Pay/Service) dt 25 Apr 60. I hereby undertake to observe and perform the terms and conditions contained therein so far. They are applicable to me in particulars to comply with the terms and conditions namely:-
(a) That the amount for within the withdrawal is applied for shall be actually utilized for purpose of building or acquiring suitable house including the site thereof.
(b) That if the amount permitted to me withdrawn is in excess the actual expenditure by me building of acquiring to gather with interest thereon at the rate provided for in Para 1 to Appx ‘A’ to the said shall refunded to the Govt from credit of my AFPP Fund forthwith in one lump sum whether the same have been demanded or not.
(c) That the purposed to be built acquired by me with amount shall be suitable at Vill: Zomi Colony PO : Churachandpur Dist: Churachandpur State: Manipur where I intend to reside after retirement.
(d) In the event of my building a house, the construction shall be completed within six months of the withdrawal of the above said amount or be completed within a period of one year from the date of commencement of construction or within such further extended period as the Govt may if selects. In the event of ready built house being purchased from private shall be repaid within three months of withdrawal of such extended periods as may be permitted by the Govt.
(e) That in the event of my building a house, the right of the sight on which the house is proposed to built will be acquired by me forthwith.
(f) The approved plans and permit where necessary from local auth for the purpose of building materials to the extend require shall be furnished by me.
(g) That in the case of withdrawal for the purpose of ready build house would secure and undisputed title to the house and the land on which the house is built before the purchased price is paid.
(h) That as long as I am in service, I shall submit every year a declaration in the form prescribe by the Govt on or before 31 Dec for the house so built or acquired to be in my sale ownership and possession.

(i) That while in service, the house so built or acquired shall not be transferred by way of sale, montage, exchange or gift or on loan for terms exceeding three years or otherwise however without the provisions of sanction with in waiting.
2. I hereby declare that my private saving together with the amount permitted to be withdrawn will be sufficient to build or acquire the house of type intended and that I don`t want house other than which is intended to be built/acquire at Vill : Zomi Colony Dist : Churachandpur State: Manipur intended place of residence after retirement. I further declare in accordance with provisions of said letter if I fail to do so, the amount permitted to be withdrawn from the fund in purpose of the said, shall be refunded to the Govt to credit to my AFPP Fund together with interest at the prescribed rate.
